Newborn model call questionnaire

Thank you for your interest in my newborn model call. Newborns under 3 weeks of age will be considered for the project. Sessions will take place in my cozy studio in downtown Everett.

In exchange for your participation, you will receive a complimentary session (no session fee - $150 value), a $100 product credit to be used towards the purchase of prints, albums and/or digital images AND a matted gift print of your choice!

Most newborn clients spend between $400 and $1200 on their portrait art however there are no purchase requirements for participants (beyond your free credit). Please do not apply if you have already booked a newborn session with another photographer (I don't want to step on any toes). :)

A fully refundable $50 retainer is required to book your session. This retainer will be completely refunded to you after the session is complete. It will only be forfeited if you cancel or fail to show for your appointment.

I understand that in order to participate in a creative session, I must sign a model release which grants the photographer the right to use and publish photos and video created during the session for any advertising, display, publication, marketing, competition, and/or other studio purposes. I also understand that for my time and participation I will receive a waived session fee, a $100 product (digital and/or print) credit and a matted gift print.
